MEDICAL & HEALTH INSURANCE REQUIREMENTS

Notice For Parents: Required Student Health Insurance

Requirements for Full-time Students
Medical and Health Insurance Requirement Checklist for all FULL TIME students:

  1. A completed Health Form signed and completed by their physician .
  2. Documentation of Immunizations on the health form as per NYS law.
  3. All students must read the medical information about meningococcal meningitis at our Web site, and complete and return the Meningitis Information Response Form. The information and form can be downloaded. Those who have a SOLAR account and are 18 years of age or older may use SOLAR to submit the response form.
  4. All full-time matriculated students must have health insurance coverage at all times without exception. Stony Brook offers a health insurance plan for all full time domestic* students that meets this requirement. This plan pays for most medically necessary bills, such as doctor visits, mental health counseling, prescriptions, emergency room, lab testing, diagnostic testing, surgery, hospitalization, etc. The plan covers our students anywhere in the world, every day, no matter whether on campus or on semester breaks.

This policy pays up to $100,000 for each illness and injury per year. Of course, there is the unusual possibility that illness or injury costs could exceed $100,000. Medical costs beyond the $100,000 limit will not be paid by this plan.

You must contact the insurance office immediately if you lose your insurance coverage for any reason (631 632-6331).
All students must review their health insurance policies every six months to determine the date when the student may "age out" of the policy. It is mandatory that the insurance office be informed as soon as you are aware of this aging-out date, so we may enroll the student in the University plan immediately. .

All full-time matriculated students at Stony Brook are automatically billed for the University Health Insurance plan at the beginning of each semester. The cost of the insurance is $403** for the Fall 2008 period.

Waivers for this insurance plan are approved only if the student already has alternate health insurance (not just coverage for prescriptions) in place, and completes a waiver on-line (on SOLAR) by September 28, 2008. Alternate insurance must be either:

  • through a job
  • through a parent
  • through a spouse
  • through some other related individual
  • through Medicaid or “Family Health Plus ”

To file a waiver the student must go to the main page of the SOLAR system, and follow the instructions for health insurance waivers under “Student Financial Services” on the menu bar. For complete information about the required health insurance plan please go to www.aetnastudenthealth.com (policy number 890444).

*Click here for information about the insurance plan for international students.

**Students enrolled in the Schools of Medicine, Dental Medicine, and certain clinical programs will be billed for Fall 2008 that covers potential clinical incidents. For more information about this additional coverage, contact the Student Health Insurance Office, or your School’s Dean of Students.

Payment of the SUNY Stony Brook Infirmary Fee—Required
The fee covers comprehensive health services for both medical and mental health problems, for students and visiting scholars. It is not a substitute for health insurance. The Student Health Service building is the only location on campus where the mandatory health fee can be used. Medical Services that are beyond the scope of the Student Health Service can be obtained either at University Hospital Medical Center or through other medical providers in the community. However, the infirmary fee will not cover the cost of any medical services outside the Student Health Service Building. Call (631) 632-6740 for further information.
