Forms for SPD Graduate Students

The only thing worse than filling out forms is driving to campus to pick them up! With this in mind, we have put our most requested forms here on our website.

• Academic Dishonesty Policies and Procedures (PDF). Revised 10/2/09.
• Academic Petition (PDF.) For adds after the add deadline and withdrawals after the withdrawal deadline.
• Change of Graduation Date (PDF). This form is required for students who have ALREADY APPLIED for graduation, but now need to change their graduation date.
• Graduation Clearance Forms. Graduation clearance forms are now submitted via SOLAR.
